Company usually makes the PE registration paperwork for the employee. You need only to get tax ID (tax number).

Now that I've been looking awhile, it seems the PE model is fairly common among IT firms here in Kiev.  Here's what a different IT company had to say about it. 

We work with IT consultants on the basis of PE (Private Entrepreneur) model. This provides lots of benefits:
–   100% salary is paid legally, which allows people to easily open credit lines with banks, receive visas and travel abroad
–   It allows us to pay significantly higher salaries (30%) v.s. traditional KZOT model
–   many IT companies utilize this model, so you will be able to easily switch to the new job (if ever decide to leave us)

We take care of all PE related overheads:
–   Pay 200 UAH per month of fixed taxes
–   Compensate 0.75% of bank cashing fees
–   Take care of all relations with Tax Administration including quarterly reporting, opening and shutting down SPD license

So it seems to be all legit.

The biggest problem with this (the PE) model is the limitation on income of a maximum of $100,000.  For most expats that is not really a problem, but you can always hope.  I setup as a registered company to allow me to better learn how companies operate in Ukraine (and, potentially, gross a far larger amount).  It has been SOOOOO much fun, paying the incredibly high payroll and social taxes in Ukraine!

I'm still going to school......

and paying tuition....

This is a good option if you want simple and a mximum cap of $100K gross revenue.  Possibly a problem if you want more than 10 empoyees as well.
